fall-risk-assessment【开发与工程】Assesses fall risk using Morse Fall Scale, Hendrich II, and Timed Up and Go from FHIR resources and generates a fall prevention CarePlan. Use when user asks to "assess fall risk", "Morse fall scale", "fall prevention", "Hendrich score", "TUG test", "fall history", or mentions patient falls, gait instability, or balance concerns. Calculate fall risk scores from FHIR Patient, Condition, Observation, and MedicationRequest resources using Morse Fall Scale, Hendrich II Fall Risk Model, and Timed Up and Go (TUG) test. Identify high-risk medications, mobility deficits, and environmental risk factors. Generate a CarePlan FHIR resource with individualized fall prevention interventions.

Extract age and gender. Age >=65 is a primary fall risk factor.
Tool: fhir_search
resourceType: "Condition"
queryParams: "patient=[patient-id]&clinical-status=active,recurrence,remission"
Key SNOMED codes:
Also search specifically for fall history:

Tool: fhir_search

Flag medications that increase fall risk. See references/fall-prevention.md for comprehensive list. Key categories:
Count total high-risk medications. Polypharmacy (>=4 medications total or >=2 high-risk medications) is an independent fall risk factor.
Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=8480-6&_sort=-date&_count=5"
Check for orthostatic blood pressure measurements (supine, sitting, standing). Orthostatic hypotension: SBP drop >=20 mmHg or DBP drop >=10 mmHg within 3 minutes of standing.
Also retrieve heart rate:
Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=8867-4&_sort=-date&_count=3"
Tool: fhir_search
resourceType: "Procedure"
queryParams: "patient=[patient-id]&code=183301007,228869008"
SNOMED 183301007 = Use of walking aid. 228869008 = Wheelchair use. Also check Condition or DeviceRequest resources.
Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=54614-3&_sort=-date&_count=1"
LOINC 54614-3 = Timed Up and Go test (TUG). If formal TUG score is documented as Observation.
Also check for gait/balance assessments:
Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=52732-7&_sort=-date&_count=1"
LOINC 52732-7 = Fall risk assessment total score (if previously documented).
Refer to references/fall-scoring.md for complete criteria.
Morse Fall Scale (Score 0-125):
Hendrich II Fall Risk Model (Score 0-16):
Timed Up and Go (TUG):
20 seconds: high fall risk, need for assistive device
30 seconds: severely impaired, dependent mobility
