Plan-only mode — inspect context and write a markdown plan into `~/.hermes/plans/` when the user wants planning instead of execution. Use when the user says "plan this", "make a plan", "/plan", "don't implement yet", or "investigate and make a plan". If the user asks to plan and then implement, save the plan first, present it for approval, then continue.
